As PCTs become commissioning-only organisations it is important that they have the freedom to choose the best care for their local populations; this is reflected in the requirement in the 2008/09 Operating Framework for PCTs to create an internal separation of their provider services. Simultaneously the healthcare landscape is becoming more competitive, with other players entering the community-based care market.
The full and formal separations of provider arms as well as the changes in the provider landscape are positive, as they will allow for greater competition in the provision of community services, leading to more choice for commissioners and better care for patients.
PCT Boards need to consider which is the best model for their provider arm? To test their readiness to become independent a diagnostic process of self-assessment for provider arms is required, including:
- can they demonstrate what they have in order to be deemed ‘autonomous’
- areas of strength as well as areas of development
- assessments of strategy, operations, people, finance, governance and quality
- scoring matrices on a scale of 5 (excellent) to 1 (inadequate), of where the provider is and what development plans are needed
