Both the 2010-11 operating framework and the health secretary’s strategy paper - NHS 2010-2015: from good to great. Preventative, people-centred, productive - have set a target for PCTs and SHAs to reduce their management costs by 30% over the next four years. Cutting procurement and ‘back office’ costs and merging management functions with other organisations, including local authorities, are specifically identified. Whilst the detail is still missing this is obviously a challenge that needs to be addressed as an immediate priority. This is even more so as NHS chief executive David Nicholson has stated that the DH wanted to ‘front load’ the cuts into year one, 2010-11: “We’d like to see up to 15 per cent in the first year if we could.”, he said.
Added to this PCTs have to come up with a strategy of what is going to be the future of their provider arms by 31 March 2010. The option of FT status is not on the table apart from a favoured few so PCTs will be exploring other options and choices including vertical or horizontal integration together with linking with a local authority or other social care provider. The optimal solution will depend on local circumstances, however the pace of change required means PCTs have much work to telescope into a very short time frame.
